Puzzle Fever is a block puzzle game that features hexagons rather than traditional squares and jigsaw-puzzle elements. The player's only goal is to correctly place shapes made of side-by-side or stacked hexagon blocks onto boards of empty hexagon spaces on each level. The game offers hundreds of levels across 14 modes of play (Basic, Intermediate, Original 1, Colorful 1, Advanced, Expert, Genius, Master 1, Master 2, Original 2, Colorful 2, Crazy 5, Crazy 6 and Crazy 7).
The player uses spatial awareness and hand-eye coordination to make the correct placements. That said, they don't need to worry if they make a mistake. The game allows them to remove a placed shape and drag it back to its original position below the board. It also allows the player to use a semicircular arrow button to reset the entire playfield.
Puzzle Fever rewards the player with a gold crown at the end of each level instead of the traditional three stars seen in most block puzzle games. It also gives the player coins they can use to buy hints that show them where to place shapes during a game.
Additionally, the game offers the player achievement rewards for completing missions, such as moving block shapes a certain number of times, receiving a specific number of "Fast!" stamps, and even completing a certain number of daily missions. Some levels offer a gift as well. The player only receives this reward after they complete the level.
