Candy Mahjong 3D is a matching game similar to Chinese Mahjong that uses cubes instead of tiles. A player is tasked with removing stacked cubes marked with the images of different treats (i.e. candies, cakes, cake pops, chocolates, cookies, ice pops and lollipops) in pairs from each level. They can only remove cubes that are at the ends of rows or standalone. To reveal matches, they can direct the stack to rotate left and right and shuffle cubes.
The player only receives 6 minutes to progress through as many levels as possible and obtain the highest score possible. After the timer reaches zero, they must start over in the first level. The game automatically turns a stack at random to throw off the player when they make fast matches.
Although Candy Mahjong 3D features these limitations, it gives the player methods for increasing the score and extending the timer. They can use a help tool to clear a pair of blocks. They receive additional points via a multiplier if they make matches every 3 seconds. If they match the same candy and dessert images consecutively, they receive x5 multiplier bonus points. Whenever they complete a level, they receive 5 seconds of additional time. If they watch an advertisement, they receive 30 seconds.
