Funny Bone Surgery is a career-simulation makeover game. The player must treat a little girl who suffered serious injuries after becoming distracted while riding her skateboard and hitting a hard metal bumper on the back of a parked car.
The player performs treatments commonly handled in urgent care and emergency room settings. After cleaning up the little girl, they tackle swelling, germs, scrapes and broken bones.
Although the game supplies many of the tools that emergency medical personnel might use in a similar real-world situation, it also contains unrealistic aspects that make it easier for younger players to understand complicated treatments. For example, the player performs extremely basic medical steps that introduce concepts like anesthesia and X-rays. They also heal wounds almost instantly and kill anthropomorphic germs with a sharp object.
Once the player completes all the medical tasks, they receive an opportunity to perform a makeover. They can change the color, style and length of the girl's hair. They can update her clothing, sling, shoe, cast or boot and cane. They can also manually add decorative accessories to her face. That said, the game offers a randomizer button marked with question marks printed on a die that automatically selects items.
Funny Bone Surgery rewards the player every time they complete a specific task. For example, the player receives recognition for cleaning away all dirt, using medical tools for the first time, curing bruised eyes, terminating germs, finishing all puzzles and trying all dress-up items and accessories.
